Contact your council’s social care department for an assessment of your social care support needs, a family member or friend can also contact them on your behalf. Your GP, district nurse or health visitor can also organise an assessment for you.
If you need urgent help let the council know because they might have a waiting list for social care support assessments.
A social worker will visit you to talk about your social care support needs.
After your assessment the social care department will write to you and let you know what help you can get.
You should be told which of the services you need are free and which you’ll have to pay for.

If you have to pay for services, the social care department may be able to help you find someone to provide them.
If you have a relative or friend who is also supporting you as an unpaid carer, they should also be offered a support plan to look at their own support needs.
