How to get social care support

If you need social care support or you want to help someone else get support, your local council’s social care department can help. The process of planning your social care support is called self-directed support.

Find out what care at home services are available.

How long you may have to wait for a service will depend on the National Eligibility Criteria.

Emergency support

If you’re coming out of hospital and need social care support, a plan will be made with you before you leave the hospital.

If you’re a carer, emergency and future planning helps you plan for the social care support of an adult in case of emergencies or if you’re no longer able to care for them.
