If you need social care support from your council you have to get an assessment of your care needs. They can offer you support such as:

• home care services
• day care
• a place in a care home
• Self-directed Support
• personal care
• meals
• getting your laundry done in your home or at a laundrette
• equipment and adaptations, like home improvements or telehealthcare
• short breaks for a relative or friend who looks after you

If you want to plan and pay for your own care and do not want help from your council it’s still a good idea to get an assessment of your care needs. This is to make sure that:
